New Patient Forms
To save time at your visit, please download, fill out and bring the following forms with you. Arrive 15 minutes before your appointment to review your patient information with our staff. Upon arrival, please have proper identification and insurance cards available for our team member at the front desk.

Appointments
Patients are seen at our office Monday through Friday by appointment. To best serve your needs, we request that you call in advance so that we may schedule a day and time that is convenient for you. However, same day appointments are usually available in an emergency – especially if you call early in the day.
To schedule an appointment, please call (407) 297-3626. If you reach our voicemail system, simply follow the voicemail instructions. All calls will be returned as soon as possible and before close of business the day of your call except for those calls received by our office after 3:30 pm. Calls after 3:30 pm may not be received by our office in a timely manner and may require a call back on the following business day.
VERY IMPORTANT: If you need to cancel or reschedule an appointment, you MUST call 48 hours in advance of your appointment time. If you do not show for your appointment, or fail to give 48 hours notice of a need to cancel or reschedule, you will be charged a fee.
Telephone Calls & Procedures
The main number at Meyer Internal Medicine is (407) 297-3626 and is answered by our receptionist during regular business hours. When you call during this time, with a problem or questions, our staff has been instructed to ask your name, phone number and the nature of your medical concern, need or problem. Your message is then relayed to the appropriate health care professional. They will return your call as soon as possible. Because your medical record may need to be reviewed by the assigned healthcare professional before calling you back, please be patient, as it is in your best interest for us to have a thorough understanding of your medical history before calling you back.
For calls regarding appointments, billing, referrals or insurance, please call our main number and ask the receptionist to connect you with the appropriate staff member. Calls for non-medical issues will be returned within 24 hours.
Prescription Refills
Prescriptions and all refills are issued during the time of your office visit. Please assess any refill needs prior to coming into our office. When calling for refills, please leave your name and a contact number as well as the phone and fax number of the pharmacy.
All requests from mail order pharmacies must be hand delivered or faxed to our office at (407) 297-3772. Requests left without the pharmacy phone and fax number cannot be processed.
Referrals
If you are calling for a referral we ask that you call our office at least 72 hours in advance of your specialist physician appointment. In order to avoid any misunderstanding, please confirm your referral with your insurance company.
Insurance
We participate with many insurance companies. To confirm Dr. Meyer and Meyer Internal Medicine are currently participating in your insurance plan, please contact your health insurance company using the phone number located on the back of your insurance card. We will collect your deductible, co-pay and fees for any non-covered services, along with any outstanding balances, at the time of your visit. We accept cash, checks, Visa, Mastercard, and American Express for payment.
Confidentiality
Your medical records are completely private at Meyer Internal Medicine. To ensure patient confidentiality and conforming to HIPPA regulations, Meyer Internal Medicine will not release information about your health or services rendered to anyone, including any family members, without your written consent.
